ACCESS FOR THE DISABLED

Manston Surgery has wheelchair access via a lift to the 1st floor, There is also a disabled toilet onsite.

Scholes Surgery has wheelchair access by arrangement at the side door. All facilities are on the ground floor.

COMMUNICATION WITH PATIENTS

Please enquire if a patient needs extra support in communication for example:
Larger print (font size can be adjusted)
Language line for patients who cannot speak English
Google language translation
Braille – via the PCT
Sign language translators can be booked in advance.

CHANGE OF ADDRESS

Have you moved house or changed your telephone number/mobile number? Please let us know so we can update our records in case we need to contact you. However please note that we will require sight of proof of address i.e. a utility bill before we change your address.

COMPLAINTS

If you have any complaints about our service, please let the Receptionist, Doctor or Nurse know. We welcome your views on how your health care is provided. For more serious complaints we do have a Complaints Procedure which meets national criteria. A leaflet is available on request.

RIGHTS AND RESPONSIBILITIES

Patients must inform the Surgery if they are unable to attend their appointment. If you persistently fail to attend, or fail to inform us that you can not attend, this will result in your removal from our list.

Violent or abusive patients will not be tolerated and will be removed from the list immediately.

Patients must inform the practice when they change their address, ensuring that we have an accurate and up-to-date telephone number.

You have the right to:

  • a courteous, efficient and confidential service which we will always endeavour to provide.
  • a chaperone present during examinations. Please ask and if it is not possible to provide a chaperone at that time we will reschedule your appointment.
  • view or have a copy of your records. There may be a cost to you for this service.
  • express a preference of Doctor and we will endeavour to meet this
  • no discrimination over age, race, disability or gender

CONFIDENTIALITY

The content of your medical records is confidential. Information may be disclosed to others who are providing your medical care, such as Hospital Doctors, Nurses, Social Workers and Benefits Agencies, but only insofar as it is necessary for them to do their job. Only with your written consent will information be provided to Insurance Companies, Employers or Solicitors.

We are registered under the Data Protection Act 1998. This came into force on 1 Mar 2000 and allows you to find out what information about you is held on computer and in certain manual records. This is known as “right of subject access”. It applies to your health records. If you want to see them you can write to us. You are entitled to a copy but should be aware that a charge will be made. You should also be aware that in certain circumstances your right to see some details in your health records may be limited in your own interests or for other reasons.
