Welcome to Crossgates' and Scholes' Surgery

Practice Services

Practice Nurse / HCA

Please make an appointment with a member of the Nursing team for a health check, blood pressure check, blood tests, dressings and other nursing procedures. Reception will advise you of the most appropriate person to see.

Test Results

Your test results will be on your computer records. Most results will take about 7 days but some may be longer. You can telephone for your results if they were ordered by the Surgery. Please call 0113 264 5455 after 10am when we are less busy and staff should be able to assist you.

Data Protection 1988

All patients’ records on file or on the computer are confidential. In order to maintain this confidentiality test results will only be given to the patients themselves or parents of minors if satisfactory identification has been obtained.

Forms

If you want the Doctor to fill in a form for you, please leave the form with the Receptionist and she will tell you when it will be available for collection. Please note that there will be a charge and again the Receptionist will advise you of this. Please do not bring forms with you for the Doctor to sign during consultations.

Minor Surgery

We perform some minor surgery operations including the treatment of warts, verrucas and joint injections. Your Doctor will advise you if your condition is suitable for treatment at the Surgery and, if it is, a special appointment will be arranged.

Maternity

Antenatal care is carried out at special clinics by our midwife. She would like to see you early in your pregnancy so please ask a Receptionist to make an appointment at the clinic once your pregnancy is confirmed.

Baby Immunisations

Immunisations are by appointment only with the practice nurse.

Cervical Smear Test

All female patients between 25 and 64 years of age are advised to have regular smear tests. This is an important screening test to look for abnormal cells, which if left untreated, might develop into cancer cells A call and recall system is operated to remind you when your test is due. Please make an appointment with our nurse either by contacting reception or booking online into a designated smear clinic.
